The department represents a clinical base for scientific subdivision of the Department of Cardiovascular Surgery.

Department of Cardiovascular Surgery of Clinic of Cardiology Research Institute was created by Full Member of the Russian Academy of Medical Sciences of the USSR V. V. Pekarsky and began working in 1987.

From that time, cardiac surgeons of the department maintain a tradition of highly skilled treatment of patients with virtually all cardiovascular diseases.

Tomsk school of cardiac surgeons, supervised by Professor V. M. Shipulin, is recognized as the leading in Russia and is awarded support grant of the President of the Russian Federation.

The department is located at floors 5 and 6 of the building 2 in Cardiology Research Institute and has 50 cardiac surgery beds as well as surgical dressing rooms and procedure rooms.

List of the treatment methods

Surgical treatment of patients with coronary artery disease (CAD). Standard method of surgical treatment for CAD is coronary artery bypass grafting: coronary artery bypass under cardiopulmonary bypass (CPB); minimally invasive surgery for coronary artery bypass via anterolateral thoracotomy approach; coronary artery bypass without CPB via standard median sternotomy; complete autoarterial and/or hybrid coronary artery bypass. It is not uncommon that, in patients with CAD, myocardial infarction is complicated by the development of the left ventricular aneurisms and ischemic mitral insufficiency and is one of the main causes of ischemic cardiomyopathy with progressing heart failure. Integrated cardiac surgeries (coronary artery bypass grafting, left ventricular aneurismectomy, and correction of postinfarction mitral regurgitation) in such patients is one of the priority objectives of the work of the department.

In the department, there is great, long-standing, and successful experience of using laser revascularization of the myocardium to improve the left ventricular function.

Surgery of valvular diseases has been developing in the department from the moment of its foundation. For reconstruction and/or prosthesis of the heart valves, the best expandable materials, manufactured by the national and world leaders in the field of producing the implants for cardiac surgery are used:

  • Prosthetic and reconstructive plastic surgery of mitral valve;
  • Prosthesis of aortic valve and reconstructive valve-saving surgeries of the aortic root and aortic valve;
  • Prosthesis and plasty of tricuspid valve;
  • Combined multi-valve reconstructive surgeries.

Patients both with valvular diseases and with CAD concomitant with atrial fibrillation can receive rhythm-conversion surgeries (maze procedure).

Surgeries via minimally invasive approach for mitral and aortic valvular diseases have been implemented. For the last two years, the area of transcatheter prosthesis of aortic valve has been developing.

Besides, the department has an experience of surgical treatment of patients with hypertrophic cardiomyopathy: myectomy with mitral valve plasty via the left ventricular outflow tract.

Surgeries in diseases of aorta and major arteries.

Department of Cardiovascular Surgery No. 1 is one of a few clinics in Russia where both elective and emergency reconstructive surgeries are performed on all segments of the aorta and the major arteries. Patients are operated both for aneurysms and for aortic dissection. Reconstructive surgeries are performed for abnormalities in ascending aorta, aortic arch as well as for thoracic, thoracoabdominal, and abdominal aorta. We have the greatest in Russia experience (one of the greatest in Europe) of the most state-of-the-art hybrid surgical treatment of aneurysms and dissections of thoracic and abdominal aorta. Reconstructive surgery of thoracic aorta is performed under cardiopulmonary bypass, circulatory arrest, protective hypothermia, and beating heart. Traditionally, the surgeries for removal of atherosclerotic plaques from carotid arteries as well as bypass surgeries for atherosclerotic damage of iliac, femoral, and infrapopliteal arteries are performed in the department.

The department has an experience of state-of-the-art single-step combined and step-wise surgical treatment of patients with oncopathology and severe cardiovascular diseases.

The department is equipped with all necessary deices and tools allowing to perform all operations in cardiovascular surgery.

During recent years, over 1200 surgeries, mostly with artificial circulation, are performed in the department. Used in the department technologies are continuously improved and, as of today, the results of surgical treatment correspond to the level of European clinics.
